School Counselor Referral form

I grew up in Mapleton riding horses competitively - from jumping to rodeos! Now, I spend my time riding through the Mountains. Driving to events with my dad, we'd listen to 70s music and I've been a fan ever since.

My inner-nerd shines with books and movies. Summer-time is filled with camping, golfing, fishing, and riding! I love to sew and am currently working on a quilt. I love detailed and technical work.

Classroom Lessons

School Counselors visit classrooms throughout the year to present Life Skills lessons. 

Possible Classroom Lesson Topics:

  • Big Emotions

  • Conflict Resolution

  • Coping Skills

  • Friendship

  • Handling Change

  • Resilience + Perserverance

  • Self-Esteem

  • Self-Regulation

Group Counseling

Students can be referred to join Group Counseling by a parent/guardian, teacher, or themselves. Students must have a signed Permission Form before they can participate. Groups run for approximately six weeks.

Possible Group Counseling Topics:

  • Anxiety

  • Emotional Regulation

  • Friendship

  • Grief

  • Perfectionism + Self-Esteem

  • Stress Management

Individual Counseling

Students can be referred to meet with the School Counselor individually by a parent/guardian, teacher, or themselves. Students must have a signed Permission Form to do so. 

Individual Counseling is on an as-needed-basis. Topics discussed in Group Counseling and Classroom Presentations can be addressed further through Individual Counseling.

School Counselors are not therapists. Group and Individual Counseling are designed to teach students skills that can help them cope with problems and/or concerns they are currently facing so they can be successful at school.
